- Lecturers
- Chair Review This form is used by the Department Chair or Program Director to evaluate 1-year part-time lecturers, or to provide an independent Chair's review when a lecturer is reviewed by a committee.
- SME-Program Coordinator Review This form is used by the Program Coordinator to provide feedback for the Department Chair in reviewing a part-time lecturers with a one semester or one academic year appointment.
- Committee Review This form is used by the department level Committee to review a lecturer with a full-time 1-year appointment or to review a lecturer's 3-year or 6-year cumulative evaluation file.
- Lecturer Evaluation The form is used by the Associate Dean or other appropriate administrator to evaluate a lecturer who has been reviewed by an appropriate committee and/or Department Chair.

Fee Waiver Program
Information on the fee waiver program, including the submission form, can be found on the .
Faculty Eligibility
- Tenured and probationary faculty unit employees (excluding coaches)
- Temporary faculty unit employees with three (3) year appointments pursuant to Article 12 of the CBA.
- Coaches must have at least six (6) years of consecutive service in the department.
- FERP (Faculty Early Retirement Program) participants are considered tenured faculty and are eligible for fee waiver only during the semesters/quarters when they are actively employed in a FERP appointment. Timebase is not a factor.
